AAOIC has published two new supplemental informed consent documents, one for patients who choose a limited or compromised treatment plan and one for patients who decline orthodontic records, including progress records. These supplemental documents are designed to be used in addition to AAOIC’s standard informed consent when either of those two scenarios occur.   Both new forms can be found in the Supplemental Informed Consent listing on AAOIC’s website:
